Order Paper CCF's in Dash

Some of the more remote drug testing sites might not have the tools necessary to accept electronic authorization forms. If you find yourself in need of paper, you can order those here.

1. Choose Request Paper CCF from the Program Actions menu

   • 

2. Select the number of forms you’d like to order. If you have both a DOT and drug-free workplace program, you’ll see options for both here.
3. Let us know where to ship it
4. Select the shipping method.
   • 2nd Day and Overnight are not options for quantities greater than 50
   • There is an extra fee associated with 2nd Day and Overnight
